Tufted Hair Folliculitis

Tufted hair folliculitis is an uncommon progressive disorder associated with cicatricial (scarring) alopecia of the scalp and can present a diagnostic challenge. A key feature is the presence of multiple hairs (often more than 6) emerging from a single follicular opening, a finding commonly linked to folliculitis decalvans but not exclusive to it. This activity emphasizes that tufted hairs reflect a late-stage scarring process in which dermal fibrosis causes follicular ostia to converge. Learners review the pathophysiology, clinical presentation, and trichoscopic features of this condition, as well as its overlap with other dermatologic disorders. The course underscores the importance of clinical context and a broad differential diagnosis. Also outlined are current evaluation and management strategies while highlighting the role of the interprofessional team in improving diagnostic accuracy and patient outcomes.
