Ulnar Neuropathy

Ulnar neuropathy constitutes a common upper-extremity compression neuropathy with multiple potential entrapment sites along the nerve s course. The elbow, particularly the cubital tunnel, accounts for most cases of ulnar neuropathy, although clinically significant compression may also occur at the wrist within the Guyon canal, as well as in the forearm or upper arm. This course outlines the etiologic factors of ulnar neuropathy, including prolonged elbow flexion, external pressure, anatomic variants, trauma, repetitive motion, and mass lesions, as well as the factors that can increase risk, such as occupational strain, metabolic disease, and previous upper-extremity injury. The pathophysiology of this condition, involving mechanical deformation, ischemia, and impaired axonal transport, producing demyelination or axonal loss, is also discussed. Participants will gain an understanding of ulnar neuropathy’s clinical manifestations, including paresthesias of the fourth and fifth digits, hand weakness, impaired grip and pinch strength, and, in advanced disease, intrinsic muscle atrophy, as well as the diagnostic processes that rely on a focused clinical evaluation supported by electrodiagnostic studies and imaging when structural causes are suspected, the management strategies, and the complications including persistent sensory loss, motor deficits, or progressive deformity. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying ulnar neuropathy, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition, ultimately improving clinical outcomes and reducing morbidity.
