Umbilical Artery Catheterization

Umbilical arterial catheterization involves placement of a catheter into one of the neonatal umbilical arteries to obtain reliable arterial access during critical illness. The umbilical artery constricts shortly after birth but remains cannulable for several days, enabling continuous blood pressure monitoring, frequent arterial blood sampling, and the administration of fluids or medications. Proper positioning requires radiographic confirmation, and ongoing vigilance reduces the risk of thrombosis, vascular spasm, hemorrhage, infection, or organ ischemia. This procedure offers substantial clinical value but carries potentially serious complications, making precise technique and careful monitoring essential for safe use in extremely premature or unstable neonates. The learning activity guides participants through relevant anatomy, depth estimation, procedural steps, troubleshooting strategies, and evidence-based management principles for umbilical arterial catheterization. Instruction emphasizes structured communication and shared decision-making among clinicians, nurses, respiratory therapists, and radiology personnel, allowing the interprofessional team to coordinate placement, confirm catheter position, and monitor for early signs of complications. Collaborative practice enhances accuracy, promotes timely intervention, and supports safer care delivery, ultimately improving neonatal outcomes and reducing procedure-related morbidity.
