Abnormal Uterine Bleeding

Abnormal uterine bleeding (AUB) refers to bleeding from the uterus occurring outside pregnancy in reproductive-aged women that is abnormal in the parameters of regularity, frequency, volume, or duration. AUB is a common condition that significantly impacts the quality of life. The PALM-COEIN system categorizes AUB into structural (polyp, adenomyosis, leiomyoma, malignancy/hyperplasia) and nonstructural (coagulopathy, ovulatory dysfunction, endometrial, iatrogenic, not otherwise classified) etiologies. Up to one-third of women will experience abnormal uterine bleeding in their life, with irregularities most commonly occurring at menarche and perimenopause. Evaluation typically includes a detailed clinical assessment and laboratory studies, with imaging and endometrial sampling as needed. Management approaches primarily treat the underlying etiology and optimize bleeding with strategies individualized based on AUB severity, cause, and patient preference. Acute, severe cases may require invasive interventions like uterine tamponade, while stable cases often benefit from medical treatments. Surgical options, including hysterectomy and endometrial ablation, are reserved for refractory cases or when fertility preservation is unnecessary. Treatment aims to address bleeding, improve quality of life, and mitigate risks such as anemia or unopposed estrogen exposure, which increases endometrial cancer risk. This activity for healthcare professionals is designed to enhance the learner’s competence in selecting appropriate diagnostic tests, managing the identified etiology, and implementing an appropriate interprofessional management approach to improve outcomes in patients with AUB.
