Anterior Interosseous Syndrome

Anterior interosseous nerve syndrome presents as a rare motor neuropathy involving the flexor pollicis longus, the radial portion of the flexor digitorum profundus, and the pronator quadratus, without associated sensory loss. Originating from the median nerve 5 to 8 cm distal to the lateral epicondyle, the anterior interosseus nerve runs deep along the forearm, making it vulnerable to inflammatory and, less commonly, compressive insults. Clinicians must distinguish true anterior interosseus nerve syndrome often attributed to idiopathic immune-mediated neuritis from isolated traumatic nerve injuries and local tendon pathologies. This course reviews anterior interosseus nerve anatomy and the syndrome’s clinical presentation, diagnostic strategies, and differential diagnoses. Conservative management remains the initial approach; surgical exploration and decompression may follow after failed nonoperative treatment, particularly in cases involving identifiable compressive lesions. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying anterior interosseous nerve syndrome, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition.
