Aromatase Inhibitors

Aromatase inhibitors are endocrine therapies used primarily to treat hormone receptor positive breast cancer in postmenopausal women. By inhibiting the aromatase enzyme, these agents suppress peripheral estrogen synthesis, thereby reducing estrogen-mediated tumor growth. The 3 aromatase inhibitors currently available in clinical practice anastrozole, letrozole, and exemestane are used in adjuvant, extended adjuvant, and advanced breast cancer settings, with additional off-label applications in selected patients. Although generally well tolerated, aromatase inhibitors are associated with adverse effects related to estrogen deprivation, including arthralgia, bone mineral density loss, osteoporosis, fractures, and vasomotor symptoms, which may negatively affect treatment adherence. This activity reviews the pharmacology, indications, contraindications, adverse effects, drug interactions, monitoring, and toxicity of aromatase inhibitors and highlights patient selection, individualized treatment considerations, management of adverse effects, and adherence strategies. Interprofessional collaboration, patient education, and evidence-based management approaches are emphasized to improve patient safety, enhance treatment persistence, and optimize therapeutic outcomes in patients receiving aromatase inhibitor therapy.
