BRASH Syndrome

BRASH syndrome an acronym for bradycardia, renal failure, atrioventricular nodal blockade, shock, and hyperkalemia is a life-threatening condition resulting from the interplay between atrioventricular nodal blocking medications and kidney injury. This negative synergy triggers severe hyperkalemia and bradycardia, creating a self-perpetuating cycle that demands prompt recognition and intervention. Although the acronym is relatively new, the association between atrioventricular nodal blockers and renal dysfunction has been recognized for decades. Atrioventricular nodal blocking agents, including -blockers and calcium channel blockers, have been used since the 1960s to treat conditions such as coronary artery disease, hypertension, and tachyarrhythmias. Despite their clinical efficacy, these agents can lead to adverse effects, including BRASH syndrome, an underdiagnosed complication with high morbidity and mortality risk. This activity reviews the pathophysiology, clinical presentation, and management of BRASH syndrome, emphasizing the need for early recognition and tailored treatment strategies. The role of the interprofessional healthcare team in diagnosing and managing this condition is highlighted to optimize patient outcomes.
