This lecture reviews the epidemiology, genetics, epigenetics, and phenotypic neurobiology of schizophrenia spectrum disorders. This review leads to the conclusion that periods of under-treatment and an increased number of psychotic relapses leads to a deteriorating clinical trajectory and loss of brain tissue, especially early in the course of illness. The second portion of the lecture then describes the elements of an effective antipsychotic trial, definition of treatment resistance, and promotes use of long-acting injectable (LAI) antipsychotics and clozapine.
