Diffuse Axonal Injury

Traumatic brain injury (TBI) remains a significant public health concern in the United States, particularly among children and young adults, with over 1.5 million cases annually. This activity provides clinicians with an in-depth understanding of the classification, clinical presentation, imaging features, and management strategies for TBI, with a focused examination of diffuse axonal injury (DAI) a severe subtype characterized by widespread white matter disruption resulting from rapid acceleration-deceleration forces. Participants will review the utility of the Glasgow Coma Scale (GCS) in TBI stratification and explore the evolving diagnostic landscape, including the roles of advanced imaging techniques and emerging tools such as computed tomography radiomics and serum biomarkers. Through clinical case insights and current evidence-based guidelines, learners will enhance their ability to recognize DAI in various clinical settings, interpret subtle radiographic signs, initiate appropriate neurocritical care interventions, and coordinate timely transfers to neurotrauma centers. Prevention of secondary injuries, intracranial pressure monitoring, sedation protocols, and the management of autonomic dysregulation are also reviewed. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying DAI, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition.
