Discoid Lupus Erythematosus

Lupus erythematosus represents a heterogeneous group of autoimmune connective tissue disorders with prominent cutaneous involvement. Cutaneous lupus erythematosus encompasses acute, subacute, and chronic forms, as defined by the Gilliam classification, with discoid lupus erythematosus representing the most common chronic subtype. This course reviews discoid lupus erythematosus, which typically manifests as well-demarcated, inflammatory plaques that progress to scarring, dyspigmentation, and permanent alopecia, most often in photodistributed areas. Although most affected individuals lack significant systemic disease, this activity also discusses the subset of discoid lupus erythematosus that can develop or coexist with systemic lupus erythematosus, particularly in disseminated disease. This course outlines the pathogenesis, reflecting a multifactorial interplay among genetic susceptibility, immune dysregulation, ultraviolet radiation, and modifiable environmental factors, as well as the diagnosis and management of discoid lupus erythematosus and related cutaneous lupus variants. Participants gain an understanding of clinical assessment, appropriate use of serologic testing and histopathology, risk stratification for systemic involvement, and stepwise therapeutic decision-making. Emphasis on early intervention, photoprotection, smoking cessation, and individualized treatment strategies aims to reduce irreversible scarring, optimize long-term outcomes, and improve patient counseling and care coordination. This activity for healthcare professionals is designed to enhance the learner’s competence in identifying discoid lupus erythematosus, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition.
