Duodenal Perforation

Duodenal perforation is an uncommon but life-threatening gastrointestinal emergency that requires prompt recognition, accurate diagnosis, and timely intervention to reduce morbidity and mortality. Peptic ulcer disease remains the most common cause, although endoscopic procedures, trauma, foreign bodies, ischemia, and malignancy may also result in perforation. This activity reviews the pathophysiology, clinical presentation, diagnostic evaluation, imaging modalities, risk stratification, and evidence-based conservative, endoscopic, and surgical management of duodenal perforation. Participants learn to differentiate contained from noncontained perforations, recognize atypical presentations, select appropriate treatment strategies, prevent sepsis and postoperative complications, and implement measures to reduce recurrence, including optimizing medications and eradicating Helicobacter pylori. Collaboration among clinicians, surgeons, gastroenterologists, radiologists, anesthesiologists, nurses, pharmacists, and other interprofessional team members enhances diagnostic accuracy, expedites treatment, coordinates comprehensive care, and improves patient-centered outcomes.
