Hepatorenal Syndrome

Hepatorenal syndrome (HRS) represents a severe form of kidney dysfunction occurring in patients with advanced cirrhosis and portal hypertension. Progressive circulatory dysregulation leads to marked renal vasoconstriction, reduced renal perfusion, and declining kidney function despite the absence of primary structural kidney disease in its classic description. Contemporary understanding recognizes that functional hemodynamic changes may coexist with structural kidney injury, complicating diagnosis and management. Updated diagnostic criteria emphasize dynamic assessment of kidney function, exclusion of alternative causes of acute kidney injury, and recognition of contributing factors such as systemic inflammation, arterial hypotension, and splanchnic vasodilation. Accurate identification remains challenging because clinical features often overlap with other etiologies of renal impairment in cirrhosis. This activity reviews evolving definitions and classification systems for hepatorenal syndrome and outlines current diagnostic algorithms used in clinical practice. Participants examine pathophysiologic mechanisms linking liver failure and renal dysfunction, including portal hypertension driven circulatory changes, inflammatory mediators, and neurohormonal activation. Clinical evaluation strategies and distinguishing characteristics between HRS and other causes of acute kidney injury in cirrhosis are emphasized. Learners also explore contemporary management considerations and ongoing controversies in classification. Collaboration among clinicians, hepatology specialists, nephrology specialists, pharmacists, and nursing professionals strengthens diagnostic accuracy, supports timely therapeutic decisions, and improves patient outcomes through coordinated interprofessional care.
