Malakoplakia

Malakoplakia is a rare chronic inflammatory disorder that most commonly affects immunocompromised individuals and results from impaired macrophage bactericidal activity, leading to persistent bacterial infection and characteristic histopathologic findings. Although the urinary tract is the most frequent site of involvement, malakoplakia may occur in nearly any organ system and can mimic infectious, inflammatory, or neoplastic conditions, making timely diagnosis challenging. This activity reviews the pathophysiology, clinical manifestations, diagnostic evaluation, histopathologic features, microbiology, treatment options, and prognosis of malakoplakia. Participants learn to recognize characteristic clinical and pathologic findings, differentiate malakoplakia from other disease processes, select appropriate antimicrobial therapy, determine when surgical intervention is indicated, and monitor treatment response. Collaboration with an interprofessional team, including infectious disease, pathology, surgery, radiology, pharmacy, and nursing, enhances diagnostic accuracy, coordinates individualized treatment, and improves patient outcomes.
