Risk & Safety Overview in Hospital Medicine Part 1

Hospital Medicine is an exciting, growing field that is dedicated to high-quality care and the principles of patient safety. However, the broad knowledge base, high-stress, fast-paced environment, task of caring for the sickest subset of patients, frequent handoffs, and co-management are among the factors that contribute to making Hospital Medicine a high-risk endeavor. The Risk & Safety Overview in Hospital Medicine courses are designed to provide clinicians with a summary of the most frequent causes of patient adverse outcomes and litigation against the hospitalist team. In addition to heightening awareness of high-risk fact patterns, the courses highlight evidence-based approaches to minimize risk and harm at the individual clinician, team, and system levels. The Risk & Safety Overview in Hospital Medicine is presented in two parts. In this activity, Risk & Safety Overview in Hospital Medicine Part 1, we cover: cognitive errors, documentation, vital signs, sepsis, acute coronary syndrome, pulmonary thromboembolism, and thoracic aortic dissection. After completing this activity, practitioners should be able to: employ strategies to minimize the occurrence of cognitive errors in order to avoid diagnostic errors in the evaluation of patients; identify signs of organ dysfunction that indicate severe sepsis and the need for early aggressive fluid resuscitation along with antibiotics in patients admitted with sepsis; and synthesize the key signs and symptoms for patients who present with severe back or abdominal pain, chest pain or focal neurological complaints in order to establish a comprehensive differential diagnosis and avoid the failure to diagnose thoracic aortic dissection.
