Secondary Stroke Prevention in Focus: Addressing Residual Thrombotic Risk

Despite advances in acute stroke management and established secondary prevention strategies, recurrent ischemic stroke remains a substantial and persistent clinical challenge. A significant proportion of patients experience recurrent events within months to years of their index stroke or high-risk transient ischemic attack, even when treated according to guideline-directed antiplatelet therapy. Current approaches to secondary stroke prevention are constrained by a narrow therapeutic window featuring platelet- and coagulation-mediated pathways. FXI/FXIa inhibition represents a fundamentally different anticoagulation strategy, selectively targeting pathologic thrombosis within the intrinsic pathway while largely preserving physiologic hemostasis. In this activity, expert faculty will review strategies to identify patients at high risk for secondary stroke, how to personalize therapy based on patient-specific thromboembolic and bleeding risk factors, and the potential role of FXI/FXIa inhibitor therapies in this setting.
