Stroke Imaging

Diagnostic neuroimaging is recommended for all suspected stroke cases, progressing from noncontrast computed tomography to advanced vessel and perfusion modalities to optimize the timely diagnosis and hyperacute management of stroke. This course reviews the distribution of cerebral arteries, which is fundamental to accurate diagnosis and management of acute ischemic stroke, including the anterior and posterior circulations, interconnected by the circle of Willis, that supply distinct brain territories, and the associated characteristic clinical deficits that occur when these areas are compromised. Participants will gain an understanding of neurovascular clinical-radiological correlation, early CT ischemic signs (including ASPECTS scoring), multiphase vessel imaging, and the identification of vessel occlusion and salvageable tissue. Advanced imaging workflows, including CT perfusion, multiphase CT angiography, and MRI mismatch are emphasized to guide eligibility for extended-window thrombolysis and endovascular thrombectomy in early, extended, and large ischemic core presentations. This activity for healthcare professionals is designed to enhance learners’ competence in applying diagnostic neuroimaging principles, following recommended stroke imaging algorithms, interpreting complex imaging findings, and implementing an appropriate approach to managing stroke.
