Gastric Neuroendocrine Tumors

Gastric neuroendocrine tumors (GNETs) are rare neoplasms arising from enterochromaffin-like cells in the gastric mucosa, accounting for less than 1% of all gastric tumors. Their increasing detection is attributed to the widespread use of upper endoscopy. GNETs are classified into 3 main subtypes: Type 1, Type 2, and Type 3, each with distinct pathophysiology and clinical behavior. Type 1 GNETs, the most common, develop in the setting of chronic atrophic gastritis and hypergastrinemia, typically presenting as small, multifocal tumors with a low risk of metastasis. Type 2 GNETs are associated with Zollinger-Ellison syndrome and multiple endocrine neoplasia type 1 (MEN1), exhibiting variable malignancy potential. Type 3 GNETs, in contrast, are sporadic, gastrin-independent, and more aggressive, often presenting as solitary, large tumors with a high risk of metastasis. GNETs exhibit variable clinical courses, ranging from indolent, localized tumors to highly malignant forms with metastatic potential. This course explores the complexities surrounding diagnosis and management, which vary based on tumor type, size, grade, and metastatic risk. Given their heterogeneous nature, GNETs necessitate an interprofessional approach for optimal diagnosis, treatment, and long-term surveillance. This activity is designed for healthcare professionals to enhance their competence in recognizing the clinical features and classification of GNETs, performing the recommended evaluation, and implementing an appropriate interprofessional management approach to improve patient outcomes.
