Somatostatinoma

Somatostatinoma is a rare, functional neuroendocrine tumor (NET) that primarily arises in the pancreas and duodenum, characterized by excessive production of somatostatin. This hormone inhibits numerous gastrointestinal and endocrine functions. Clinically, this condition often presents with a classic triad of diabetes mellitus, cholelithiasis, and steatorrhea, resulting from hormonal suppression. Although NETs are increasingly diagnosed due to improved imaging techniques, somatostatinomas remain exceedingly rare, accounting for less than 5% of functional pancreatic NETs. These tumors may occur sporadically or in association with hereditary syndromes, eg, multiple endocrine neoplasia type 1, VHL, and neurofibromatosis type 1. An interprofessional team approach is essential for optimal management, including early recognition of potential genetic syndromes and individualized therapy to balance disease control and quality of life. This course explores the complexities surrounding this condition, including the stepwise diagnostic approach involving clinical suspicion, elevated fasting plasma somatostatin levels, imaging studies, and histopathological confirmation via biopsy, as well as treatment, which is guided by tumor location, grade, and stage. This activity is designed for healthcare professionals to enhance their competence in identifying somatostatinomas, performing the recommended evaluation, and implementing an appropriate interprofessional approach when managing this condition.
